New Patient Information

ALL Patients are required to present a current and enforced PHOTO ID upon each Office Visit…there are absolutely NO EXCEPTIONS to this policy.

New Patients are taken on a walk-in basis.

New Patients must be signed in no later than 30 minutes prior to closing hour, in order to insure that our Physician and Staff have adequate time to invest in your initial office visit.

NEW PATIENT PAPERWORK

One of the most critical points to qualify as a patient with our Practice is to meet the BMI Standards for treatment. Please refer to the BMI Chart below to determine if you qualify as “Overweight” or “Obese”.
